POSITION SUMMARY

A professional nurse registered in the state of Maryland responsible for delivery of patient care through the nursing process of assessment, planning, implementation and evaluation.  Also directs and guides patient teaching activities and ancillary personnel, while maintaining standards of professional nursing.

DUTIES/RESPONSIBILITIES

  1. Assesses physical, psychological, and psychosocial dimensions of special needs patients upon intake referral for assistance with placement and continuity of care upon release.
  2. Applies knowledge of illnesses, injuries and diseases using available resources in obtaining a health history, and in the assessment process.
  3. Provides a written assessment of patient in first 14 days of admission upon referral from intake nurse as a “special needs” patient for discharge.
  4. Plans an individual treatment program for continuity of care upon release.
  5. Coordinates release planning activities with HCSO Mental Health staff and Department of Health HIV program, as appropriate; works with ADAP to ensure medication continuity upon release for HIV patients.
  6. Utilizes referral process to appropriate health care providers as necessary, both internally and externally.
  7. Applies knowledge and utilizes available resources in planning care and patient teaching for self-care upon release.
  8. Consults as necessary internally with other nurses, nurse practitioners, physician assistants, physicians, correctional officers, etc.
  9. Implements treatment plans as ordered by the physician for discharge planning and after care.
  10. Intervenes with proper techniques, procedures and available resources.
  11. Implements nutrition and therapeutic diet plan as ordered by physician or physician assistant upon release to include patient education.
  12. Documents nursing encounters utilizing the Problem Oriented Medial Records format of charting as required by the Facility.
  13. Communicates information to other nursing staff, Health Care Unit physicians, physician assistants, Health Care Unit supervisory personnel, and department correctional officers and departments.
  14. Respects dignity and confidentiality of patients.
  15. Coordinates continuity of care with community-based organizations; participates in community-based activities as representative of Wexford with the HCSO.

LICENSING: Currently licensed as a Registered Nurse in the state of Maryland

CERTIFICATION: Current CPR certification or ACLS

PREFERRED EXPERIENCE: A minimum of one year clinical experience preferred.

EDUCATION: Graduate from an accredited School of Nursing
