POSITION SUMMARY

The Discharge Nurse is a licensed registered nurse (RN) responsible for ensuring coordination of discharge planning services and continuity of care for patients upon release from a correctional facility.  He/she utilizes interdisciplinary/multidisciplinary approach to coordinate the discharge process to include, internal and external resources that will assist with potential barrier to release, patient education about continued care, and transitioning back into the community.

DUTIES/RESPONSIBILITIES

  1. Upon intake referral, assesses physical, psychological, and psychosocial dimensions of special needs patients for assistance with placement and continuity of care upon release.
  2. In the assessment process, utilizes available resources and apply knowledge of illnesses, injuries and diseases to obtaining patient’s health history.
  3. Upon notification of release date, the discharge nurse will interview the patient to assess needs and possible barriers to release.
  4. Upon release, provides patient with an individual release plan for continuity of care.
  5. Coordinates release planning with community agencies.
  6. Utilize referral process to coordinate care with appropriate health care providers both internally and externally.
  7. Applies knowledge and utilizes available resources in planning care and teaching patient about continuity of care.
  8. Consult with internal nurses, nurse practitioners, physician assistants, physicians, correctional officers, as necessary.
  9. Review Continuity of Care form.
  10. Conduct a follow-up interview with the patient to assess clear understanding of release plan and provide patient education regarding diagnosis and medications thirty days prior to release date.
  11. Coordinate continuity of care with community-based organizations; participates in community-based activities as representative of Wexford.
  12. Provide a log/record/ or tracking system to reference when patients are released and report to the Statewide Re-Entry Coordinator
  13. Attend monthly meetings with interdisciplinary team and other discharge nurses.
  14. Develop and maintain relationships with referral sources (current and new).
  15. Respects dignity and confidentiality of patients.

ADDITIONAL DUTIES AND RESPONSIBILITIES

  1. Assesses personal and learning development needs.
  2. Plans and implements program to meet identified needs through in-service attendance and other format and informal means.
  3. Accepts staffing assignments, and performs other related duties as required or assigned by the Statewide Re-Entry Coordinator.  Performs within the normal limits of the State Nurse Practice Act, Code of Ethics for Nurses, policies and procedures and facility policy/procedures and standards
  4. Develops and maintains professional relationships with patients, visitors, community liaisons, and medical and correctional personnel.  Promotes good working relationships among staff and between other disciplines and departments.
  5. Identifies existing and potential problems, and takes effective action to ensure after care services.
  6. Develops and maintains an awareness of needed improvements in patient care, procedures, equipment and supplies, and makes recommendations to supervisors.
  7. As requested, assists in the presentation of in-service education.
  8. Assumes individual accountability for own conduct and maintenance of professional appearance.
  9. Conforms to Wexford Health Sources, Inc., and Facility regulations as appropriate, to include, but not be limited to:
  • Dress code
  • Schedule/time sheet
  • Safety and security regulations
  • Procedure for sick leave/vacation time/education seminars
  1. Maintains the confidentiality of inmate records and compliance with HIPAA privacy requirements.
  2. Approaches change in a positive manner.
  3. Makes suggestions for improving work conditions to Statewide Re-Entry Coordinator and Director of Nursing.

ENVIRONMENTAL CONDITIONS

Standard medical setting within a correctional facility, which includes daily interfacing with offenders/patients, correctional staff, and health care personnel. Temperature extremes may be encountered while moving throughout the facility outdoors and indoors in buildings without air conditioning.


 

TRAVEL: Minimal travel throughout region and state is required

LICENSING: Currently licensed as a registered nurse within the state of Maryland

CERTIFICATION: Current CPR certification or ACLS

EDUCATION: Graduate from an accredited School of Nursing

PREFERRED EXPERIENCE:

  • Prefer a minimum of one year clinical experience
  • Correctional Experience preferred

  • Strong interpersonal skills
  • Strong verbal and written communication skills
  • Demonstrated experience meeting multiple deadlines. Must ensure documents are ready for review well in advance of any deadline.

  • Must be a "team-player" as this position requires daily coordination with other coworkers.
