POSITION TITLE: Regional Hepatitis C Nurse Coordinator

DEPARTMENT: Hepatitis C Elimination Project

REPORTS TO: Statewide Hepatitis C Clinical Leader 

LOCATION : Central NM Correctional Facility in Los Lunas, NM
 

The Regional Hepatitis C Nurse Coordinator plans, develops, directs, implements and evaluates plans associated with the Hepatitis C Elimination Project within a number of correctional facilities. This position will work closely with the Statewide Hepatitis C Clinical Leader; communicating patient information to the site Providers and following through with Provider recommendations. 

ESSENTIAL DUTIES/RESPONSIBILITIES

• Coordinates/facilitates the Hepatitis C Elimination program at multiple New Mexico correctional facilities. Collaborates with facility staff and coordinates internal projects, as directed • Serves as a point of contact for all Hep C patient questions, acts as a liaison between the patient and provider. • Ensures a coordinated patient HCV treatment, monitoring and follow-up program. • Coordinates patient care with providers. • Proactively contacts Hepatitis C patients. • Develops innovative teaching methods and plans for teaching the concept of the goal of the program, HCV elimination. Maintains documentation on Hep C trainings. Coordinates a series of educational classes to meet the needs of the Hepatitis C population and the on-site health care teams. • Acts on recognized trends of Hepatitis C to establish control programs. • Participates in committee discussion related to HCV and offers suggestions and inventions as necessary. • Develops policies and procedures using the literature and the national guidelines. • Communicates with staff about Hepatitis C problems, provides literature to support advice and action problems to establish protocols. • Uses medical literature to convey the message of establishing HCV treatment protocols. • Reviews literature and uses key information to develop data collection form; refines data collection process as the process unfolds; and presents data for trending analysis with proposed interpretation. • Analyzes HCV infection surveillance data, performs statistical analysis, and analyzes information during discussions. • Provides consultation to the sites and takes problem solving action. • Documents and identifies problems, initiates memos, fliers, and educational material. • Discusses problems involving outside agencies with administration and recommends plans to resolve. • Participates in department meetings/CQI/Hepatitis C committees and establish action plans, as needed. • Coordinates departmental projects. • Adheres to internal controls established for department. • Maintains confidentiality of inmate records. • Coordinates with Project Echo to help guide the HCV elimination program. • Ensures compliance with all Hepatitis C activities required by the RFP/Contract and the Hepatitis C Elimination Project. 

Job Requirements:

LICENSING: Current New Mexico Registered Nurse license required

CERTIFICATION: Current CPR Certification and/or ACLS certification required

EDUCATION: Graduate of a School of Nursing

PREFERRED EXPERIENCE • Minimum of two (2) years of clinical experience preferred • Correctional experience a plus

OTHER PREFERRED KNOWLEDGE, SKILLS AND ABILITIES • Strong interpersonal skills • Strong verbal and written communication skills • General math and analytical skills, and planning, administrative and organizational skills essential • Strong technical experience and proficiency with Microsoft Office programs, including Outlook, Word, Excel, Access, etc. • Possess cognitive skills necessary to understanding terminology, medical records and instructions • Possess skills in establishing and maintaining effective working relationships with patients, staff and custody • Possess ability to organize and carry out work assignments with minimal supervision and to prioritize work assignments and departmental responsibilities • Strong researcher in infection prevention and control • Demonstrated experience maintaining/meeting multiple deadlines. Must ensure documents are ready for review well in advance of any deadline • Ability to competently manage multiple priorities simultaneously • Ability to work in a stressful environment • Must be willing/able to assist staff with various policies, administrative and organizational projects; be a “team-player” as this position requires daily coordination with other coworkers
